- I am getting a “System Discharge” error when trying to complete my Annual Survey. How do I reconcile the “System Discharge” error?
- How do I check the status of my Annual Survey?
- Launch the Additions During Survey Period report with columns ready to filter/sort
- Select the date row (last row), right-click and select Delete to delete the row
- Select Row 3, click on Sort & Filter in the main menu and select Filter
- Select the Filter/Sort button (down arrow) in Column N (Discharge Reason). The Filter/Sort dialog box displays
- Click the Select All box to deselect all items, then click the System Discharge box
- Click the OK button to run the filter
- When the filter is run, only patients with a Discharge Reason of System Discharge display
- Note the Patient ID (CW UPI) for these patients, and then go back into CROWNWeb and review the patient’s Admit/Discharge Summary record
- System Discharges occur when a patient is transferred into a new facility without being discharged from the Home facility. In that patients’ admit/discharge record, edit the Discharge Reason to Transfer
- If a transient patient was not admitted with a Transient Status = Yes, CROWNWeb adds a System Discharge to the previous admission record
- Once the System Discharges have been corrected, remember to go back to your facility annual survey and regenerate to reflect the changes you made
• Click on the Form 2744 from the Home screen • Click on Status Change Updates in the Sub-menu • This will show if your Annual Survey was rejected, accepted, or finalized • If your survey was rejected, view the comments that was entered by the Network contact • Resubmit your Annual Survey when errors are resolved
