First Admission Not New ESRD

Within the CROWNWeb website it is impossible for a user to create an initial admission with the admit reason of “Transfer In”. However, 80% of the facilities are Batch Submitting Organizations (BSOs) such as DCI, DaVita, FMC and NRAA affiliated units. Some batch submissions do create a situation where the first admission is “Transfer In” and not “New ESRD”.

When this happens CROWNWeb is tricked into thinking a 2728 form is not needed, so you will not be prompted in any report to submit one, and if you try before correcting this, CROWNWeb will not let you create a 2728 form.

To fix this you must change the admission reason to “New ESRD” in BOTH your electronic medical record AND CROWNWeb, and then complete the 2728 form.
